Downstairs is the exam the knee cannot fake
People will tolerate a walk. They will tolerate a gym session that stays in a friendly range. Then they hit the car-park stairs at Westfield or the back steps at home and the knee announces itself. Pain going down is one of the most specific complaints we hear at Evolve Physio & Mastery — Cabramatta, Liverpool, Fairfield, Bankstown.
Upstairs is mostly concentric. Downstairs is eccentric control plus higher patellofemoral load. If the quad has been quiet since a sprain, a scope, or a month of avoiding steps, descent is where the gap shows.
What it often is
- Patellofemoral pain — the classic “under or around the kneecap” on stairs. See runner’s knee.
- Quad inhibition after injury or surgery — the thigh that looks fine and does not fire. Common after meniscus and ACL stories.
- Osteoarthritis — stiffness, older knee, worse the first flight of the morning.
- Tendinopathy — pain just below the kneecap that warms then complains on the way down. Heavy-slow protocol.
What we actually change
We do not prescribe “never use stairs.” We reduce the daily flight count that is flaring you, then we train the lowering job: sit-to-stand, step-downs from a small height, heavy-slow quad work, then faster and higher steps. Handrails are a tool, not a moral failure.
If the knee swells after a day of shopping-centre stairs, that is data. Treat it like a session that was too hard — effusion rules — then rebuild.
A week of not making it worse
- Take the lift for the extra floors this week if you have a choice. Keep one short stair practice on purpose.
- Every day: sit-to-stand and a small step-down you can do without next-day puff.
- Do not stretch the front of the knee aggressively into the painful angle “to loosen it.”

Why does down hurt more than up?
Descent is a controlled fall. The quad works eccentrically to lower you, and the kneecap sees higher compressive load. If the quad is shut down or the joint is already irritable, downstairs is the first task that fails.
Is this arthritis?

Should I avoid all stairs?
Avoid the painful dose for a short window, not the rest of your life. We find a step height and a speed you can do without next-day puff, then build the eccentric quad.
Does a puffy knee after stairs mean I should stop walking?

Will a brace fix it?
