Months matter — but passing gates matters more. Below is a typical month-by-month map we use at Evolve Physio; your surgeon’s protocol always comes first.

Month 0–1: Calm the knee, wake the quad

  • Swelling and extension priority
  • Quad sets, straight-leg raises as allowed
  • Gait normalisation
  • Patella mobility

Month 2–3: Strength foundation

  • Progressive closed-chain strength
  • Bike / elliptical conditioning
  • Balance and control
  • Prepare for jog criteria

Month 3–6: Run and hop build

  • Walk-run then continuous running if criteria met
  • Hop progressions
  • Change-of-direction introduction late in this window
