Readiness checks before first jog
- Surgeon clearance — explicit approval for impact / jog in writing or protocol
- Full extension — no flexion contracture; heel height equal standing
- Swelling — minimal effusion; no daily puffiness
- Quad activation — solid single-leg squat without knee cave or pain
- Strength symmetry — target ≥80% limb symmetry index on key tests
- Gait — normal walking pattern without antalgic limp

Week-by-week walk-run protocol
Weeks 1–2: Walk only + gym strength
- Fast walking 30–40 min, 4×/week
- Continue ACL strength block — squats, RDLs, step-ups as protocol allows
- Bike / elliptical conditioning
Weeks 3–4: Introduce jog intervals
- 30 sec jog / 2 min walk × 8–10 cycles, every second day
- Flat surface; focus on quiet landings
Weeks 5–6: Extend jog intervals
- 1 min jog / 1 min walk × 10 cycles, 3×/week
- Monitor swelling within 2 hours post-session
Weeks 7–8: Continuous easy jog
- 8–12 min continuous jog at conversational pace
- Add hop prep in gym — not on sore days
Weeks 9–10: Build running volume
- 15–20 min jogs; ≤10% weekly volume increase
- Introduce straight-line strides only if hop tests progressing
Weeks 11–12: Toward normal training
- 25–30 min runs; plan return-to-sport testing timeline
- No cutting or sprinting until hop and strength batteries pass
Strength work you cannot skip
- Quad and glute — split squats, Spanish squats, hip thrust patterns
- Hamstring — Nordic progressions as protocol allows (graft-dependent)
- Single-leg control — step-downs, single-leg RDL
- Hop progression — double-leg → single-leg once running is clean
- Trunk and pelvis — anti-rotation work for landing stability
Red flags — when to step back
- Effusion within 2 hours of every run session
- Sharp graft-area pain or giving-way sensation
- Loss of full extension after running
- Limping during jog intervals
- Surgeon or physio holds impact — honour the pause
