5 Best Exercises for ACL Rehab
Rebuild a knee that can run, jump, cut and play again
Five ACL rehab exercises we use after surgery: sled, knee extension, landing, deceleration and single-leg work — plus the details that make them useful.
If your goal is a knee strong enough to run, jump, change direction and get back to sport, these are five exercises we want somewhere in your recovery.
One of our clients, Linda, came in after ACL and meniscus surgery. Two goals: get back to martial arts, and play with her kids without the knee constantly holding her back. She worked her way toward both.
This video is the five exercises we use to rebuild strong knees after ACL surgery — and the small details that actually make them useful. They are not week-one post-op work, and they are not a complete program on their own. They are the gaps we keep seeing when someone has “done physio” and still doesn’t trust the knee.
The five exercises
- Sled — heavy quad work without hammering the patella tendon. Stay in a small range; don’t lock out into extension so the quad stays under tension. Range is a tool. Tension is the goal.
- Knee extension — straightening after ACL is scary. Use the machine: 45° isometrics, then 0° isometrics, then eccentrics. Holds before the weight starts moving.
- Landing and load absorption — strength is not enough. The knee has to accept force. Start without a box, add a box, then single-leg. Avoidance keeps the fear rent-free in your head. The more you avoid it, the stronger it gets.
- Deceleration — learning to slow down. Cone drills: run, then brake. Shorten the distance or increase the speed as you improve. One variable at a time. Sport punishes the people who only trained the go.
- Single-leg work — running, cutting and landing are single-leg dominant. Calf raises, squats, machine work — one side at a time. Two-leg squats let the strong side cheat.

The part most programs skip
You can do every exercise in this video and still not be ready to return to sport. The body progresses. Mentally you still see yourself as injured. Someone says “you’re ready — go play,” and the first thought is “what if I do it again?”
