About our ndis physio service
NDIS physiotherapy helps participants work toward the goals in their plan — whether that is walking more confidently, transferring safely, managing pain, recovering after surgery, or building strength for daily activities. At Evolve Physiotherapy & Mastery we are a registered NDIS provider for Therapeutic Supports.
We work with self-managed, plan-managed and NDIA-managed participants. Our team is experienced with neurological conditions, cerebral palsy, stroke recovery, intellectual disability, complex musculoskeletal needs and post-surgical rehabilitation linked to a participant's primary disability.
Sessions are tailored to the individual — not a generic program. We coordinate with support coordinators, plan managers, families and other allied health professionals. Funding reports for plan reviews are prepared at no additional cost when clinically indicated.

Is Evolve Physio a registered NDIS provider?
Yes. Evolve Physiotherapy & Mastery is a registered NDIS provider for physiotherapy services. Self-managed and plan-managed participants can choose us directly. NDIA-managed participants can access our services where we hold the appropriate registration category.
Which part of my NDIS plan pays for physiotherapy?
Most NDIS physiotherapy is funded under Capacity Building — Improved Daily Living. Sessions, reports, and in some cases travel to home visits are billable items under the NDIS price guide. We explain all costs upfront in your service agreement.
Do I need a GP referral for NDIS physio?
No GP referral is required. You need a current NDIS plan with relevant capacity-building funding, or approval from your plan manager or support coordinator. Bring your plan or plan details to your first appointment.
Can you do home visits for NDIS participants?
Clinic-based sessions are our primary model at Cabramatta, with good parking and accessibility. Home visits may be available depending on location and clinical need — ask our admin team when booking.
Can I switch NDIS physio providers?
Yes. You are not locked into one provider. If you are plan-managed or self-managed, you can change at any time. Give reasonable notice and ensure any outstanding reports or handover notes are completed.
Do you prepare reports for NDIS plan reviews?
Yes. When clinically appropriate, we prepare progress reports and funding recommendations for plan reviews at no additional charge beyond what your plan allows for reporting time.
